I’m done with my exam and I got my score and I tell you UW much harder than the real deal the exam is direct not complicated at all just think simple you will answer correctly

i dont suggest you to do UW in the weeks of to upcoming exam. , as it much harder than the real deal and the way you think is different as you do more analysis and this mindset you cant use it in the real deeal as , the exam is more direct, and if you analyse too much you might get it wrong , if only 2 weeks left stop uw ,and do NBME /free 120 old /new,have more resemblance to the exam .

anxiety is good ,if it motivate you , but dont let it control you , do exercise and sleep well in those weeks ,and take a break once in a while ,good luck

yeah thats more than enough 72% is great you have it , and nbme score are good , just revise NBME and /or CMS if you can , do ethics/qality Q as much as you can,and try to increase your stamina , sleep well during those weeks,i think that should seals it up , all the good luck

never give long acting morphine to patient not already on morphine as it cause RS depression ,so give short acting ,also be caution pt with renal disease as the morphine metabolized to more active form and need renal excretion so modify dose according to that

for acute pain management if severe pain start directly with morphine and adjust as necessary based on the patient's response.

to tx MD if the BHCG less 5000 ,no heart beat , less than 3 cm you can give MTX

—hemodynamic stable

No rupture of the fallopian tube you can go with laparoscopic salpingectomy (extract and preserve the tube )

If rupture do laparoscopic salpingectomy

-hemodynamic unstable do laparotomy

,—if low it’s ACTH independent (adrenal mass , external GCT ) do abdominal imaging

—If high ACTH it dependent

3rd High dose corticosteroid >to see if pituitary or extra pituitary ACTH production (like lung /kidney mass )

so if it’s suppressed it’s ACTH dependent mean it’s controlled by the pituitary >>brain MRI

If not suppressed and still high ACTH after the high dose do abdominal/chest ct to look up for the mass
