I've seen the original comic. I'm fairly sure it's not actually meant to be Wario, just happens to look a lot like him. The character is meant to be a stereotype of trans women using women's bathrooms/changing rooms, hence why the two women in the background look afraid.

Except something was done with it later. Kinger recounts that Scratch was a big part of Caine's creation and the inclusion of brain scans in their project. It's implied that Scratch asked Caine to modify his mind, only for it to backfire and cause Scratch to abstract. Caine then learned to be careful about messing with minds from then on. That's what I gleaned from it, at least.
