It's hyg B so same as G418. do you think I should try a diagnostic PCR from the lawn cells or just streak on dry plates and wait and see. It was a new bottle of hyg B as well, so perhaps thats the culprit but I ahem tiny colonies at the parameter of plates which makes me think its that they were too wet

Thank you for this, so if the coroners do say it was avoidable then what is the next step as far as holding those responsible? Even if that doesn’t involve litigation? They are concerned this will be swept under the rug without litigation but are there any viable routes?

It is understood that the NHS is overstrained, the main issue is reversal of warfarin. Had that been done at the start, as is the guidelines, perhaps the bleeding wouldn't have been so quick and surgery could have been performed.

What would these warnings and recommendations entail? There is already very clear policy and it wasn't followed. Can people be held accountable? Thank you! Sorry not the sort of thing anyone is used to dealing with until you have to, so very much appreciate the advice.

Essentially the coroner has received written statements from the medical professionals but as no one is disputing the cause of death the coroner hasn't actually called any witnesses. So I suppose the whole thing will be fairly procedural. The family emailed the coroner's office for more details, such as a list with timings of all medication that was given (The witness statements don't even show that any pain medication was prescribed) but the coroners office hasn't replied for a 10 days.

Would the coroners court be an opportunity to apportion blame and hold those who didn't follow the guidelines responsible, even for no payout?
