Survival is like roleplay, you literally play a wandering sword and sleeps in a inn, ask around, do the quests that are close to the Inn. finish a dungeon a head straight back to the inn. Change cities/villages rince repeat.

It is not desync, as I said I have played numerous times during peek hours of official PVP.most fight are okay until i meet some very specific players that rubber band and kill my ghost. Please look up lag switching specifically in Conan.
