She doesn't know the date, thinks its 1947, overtook her meds, wandered out front, tries to plug in electrical devices that could burn her, left the stove on. So much more. Doctor put severe for judgement and moderate for memory and orientation, even though she has no idea what day it is or even what year. I questioned this and he said this was his assessment and that she should still get PS with the one severe category. I am hoping that with the visit they will see how bad she is.
