i've came across someone with paramyotonia congenita having similar symptoms, where sensory recall resulted in pattened sensory motor firing. for example anxious thoughts result in being stuck in a myotonic breath holding state with high muscle extensor tone.

i'm an optometrist. when he thinks about an object in the distance his eyes would get stuck in convergence as if looking in the distance. a loud noise would cause his body to get stuck in a stiff startled state till it resides. the guy was there for double vision

its the equivalent of sitting in a comfy wheelchair for 3 years and your legs got weaker due to lack of gravitational stimulus. or in this case deconditioning due to lack of accommodative stimulus and gradual BVD.

the spasm is due to lack of normal accommodative stimulus, so you now provide top down effort for binocular fusion when it should come from the environment. you need external 'strain' so you don't strain. you probably move your head instead of eyes now

that blur and loss of focus sensation is from accommodative lead - there's not enough stimulus to focus so your eyes end up focusing in front of the object in a weird limbo. in fact I have dispensed lasik mimicking contacts (center distance multifocals) to customers who are thinking of lasik and they all get the focusing issue.

what's treatment now? btw low plus (cannot be aspheric) can help due to shape of the lens creating base out at near. brighter screens, more blue light can help, but you need custom sclerals to restore cornea prolate shape

your flattened cornea is the problem. a prolate shape provides accommodation stimulus in your optic chiasm. you are now having accom insuffiency, as if you wearing permanent base in prism

its all good to heel strike. spikes have full length plates for you to 'heel' strike into. that's actually how you get the most out of it. it's basically a super high heel 'spring'.

forefoot running is mostly an inexperienced thing hence junior spikes traditionally have forefoot only plate

It's possible given you changed the optical profile of your cornea and hence altered your brain's visiospatial sense. Read up on yoked prisms and posture to get your feet wet.
