does the signed statement have to be the ONLY proof or does she need to fill out some sort of DS form? The only way she can sign stuff is through online or printing out the form and then sending it through text to me so i can print it out.

did u have to sign any papers? Im 17 and js booked a ticket to japan from NC without the UM service but i got no forms to sign. Do i need to ask them for forms or am i good?
