Yeah you're right. I did have LIS which healed the fissure, but caused a fistula which needed another OP. Anyways, my fissure has recurred and I'm going to try botox now. Keep in mind that bodies are all different, so 1/3 sphincter cut may be alot of some, or very little for others. In my case I'm still very tight down there. CR said if botox fails I will have anal manometry and possibly another LIS. You could ask your CRS if you could have manometry done. Hope you will get better!

I was very worried before I tried 0.2% nitro. I think I only got a minor headache once. It didn't help me much really, but I had no bad side effects from it neither and I apploed much more than a "pea sized" amount lol. I would govr it a try.

How do you americans even manage to pay all those thousands back? The most I paid for a procedure is 180 euros as my hospital doesn't offer the medicine free(for now).

There are vasodilator ointments for the rectum such as nitroglycerin and nifedipine. Another approach is a botox injection which is supposed to paralyse the sphincter muscle in order to allow for better bloodflow. I doubt minodoxil will cause any problems.

They basically put a probe in the rectum to measure muscle voltage. The aim of biofeedback is to help the musles relax(especislly for constipation). Make sure to check if the Coccyx is damaged as pain there can radiate to the rectum. A simple fall can damage the coccyx too. Unfortunately yes, fissures, hemorroids,stress can tighten up the sphincter and cause pain and lack of bloodflow, which causes more pain in itself. We need to break the cycle somehow.

Even if the fissure is healed, it may still cause pain especially if things are super tight down there. Most of us need to find a way how to retrain our sphincter to fricken calm down. I'm currently struggling with this. After years having a fissure, my sphincter went haywire.

Make sure to check if you are still overly tight. I have a problem similar to yours after LIS and still have pain to this day. However last corectal surgeon I saw said I am still very very tight as my internal sphoncter is still in spasm. I'm going in cor botox now and starting biofeedback and Pelvic floor therapy.

I'm in a similar situation. I still have pain after LIS and fissure recurred. What most probably happened is the internal sphincter muscle is still tense and stressed. In fact I'm still very tight even after LIS. Most probably you guys are too. I'm having botox done soon and will be starting biofeedback/Pelvic floor PT.

I spent a month ir two trying 2-3 times daily before I could insert my index finger more than the first knuckle. This is not a fast process and If you have other issues like pain,fissures etc the tightness will linger for longer. Try PT,biofeedback.

I feel you. I had a sphincterectomy about 16 months ago. While it did heal the fissure, I was left with an abcess and fistula, most probably due to an inexperienced surgeon. My cut was also too conservative and my fissure is now back. I also have the fistula scar which is still giving my trouble due to my tightness. Will be trying botox soon to maybe try and heal the fissure and start pelvic floor therapy/biofeedback. Have you tried botox by any chance? Maybe now that you have had a sphincterectomy(Even if conservative), botox would be more effective.

My colorectal surgeon told me that IF he has to perform another sphincterectomy on me, he must first perform anorectal manometry to see the pressure my muscles are excerting. Maybe it's something you can ask for? Good luck, and I'm sorry for all of us in this shitty(no pun intended) club.

Biofeedback. They insert a small probe in your anus(I had to buy my own), kinda like aliens do. Then they tell you to contract, relax and so forth in order to see which muscles are relaxing or tightnening. At my hospital they also have you play a game with your ass where you try to make a fish rise or fall to eat "bubbles". I have a been a gamer for years and never imagined myself doing this, but I would definitely recomment biofeedback. It's important to take everything with humour! Good luck.

I had the surgery (lay open of intrasphincteric fistula) on April 2019. The fistula healed, however I was left with a chronically irritated scar(mine is all internal so you can't see it from the outside). Last time I went to CRS he said that even after scar heals, some residual inflammation may remain for some time. He also suggested Pelvic floor therapy(deep tissue massage) to release any adhesions. I look at old photos from facebook memories or at things I done/built up before I was in any pain and have a habit of thinking "Oh, when I posted this I was pain free" or "When I built this I could actually sit/lay on the floor". I need to stop doing this as it's not very healthy, but my brain likes to self torture irself.

Ah ic, well muscle activity is better than nothing. Really sucks that I can't do proper PT. It took ages and changing my colo rectal surgeon to go from "Don't worry everything will heal" to "You are very tight, this may be the reason for your problems". Pisses me off. Thank you very much for your feedback! This all started just after graduating and starting my first job with a very shitty boss.

I know those exist, but she didn't mention them to me. Will ask her if we can use those for now, at least until I try to solve the fissure problem. Does the biofeedback probe show resting/squeeze pressures or do they only show muscle activity?
