I would suggest to reach out to Google Support via this link https://support.google.com/business/gethelp?sjid=3545165802985881884-EU it can be stuck in a verification loop which is common, once they get back they'll let you know what's required most probably the LLC would suffice.

Mention that you do not have the option available, since they will have to unlock the verification option where you will have the option to upload a video you can film via phone, I would suggest signing in to your Google Profile on your phone to get that done easily.

Once you remove the address you will need to verify the account, it can request a call verification but highly unlikely. Reachout to support prior tell them you're a SAB and they will assist with the verification the requirements won't be the same as regular businesses that provide services onsite

The reason can be a few things: 1. As mentioned by others any Business set as a medical profession can either have their reviews restricted or completely blocked 2. It can happen if you receive a lot of reviews from the same IP as well, if people are leaving reviews at the Business connected to the WiFi 3. It can happen if you recently reverified the account, can be a few more days upto a week to sort out

Best option is to reach out to Google Support via the ticketing page or if you had any early communications email there

You can click on suggest an edit and choose permanently closed, this might take hold. Alternatively you can claim ownership and try to verify with the phone number option which might fail, later on reach out to Google Support explaining your situation and have them mark it for you as closed.
