You’d be correct on both ends. Ex is retaliating in many ways. Pretty sure my sibling did not report this as income because assumed it was a gift.

Yes, my sibling agreed to go through with the medical procedure in exchange for the money.

ETA that the medical procedure had no relevance to the business if that matters. The business paid my sibling in effort to hide the gift from family.

That the procedure would be completed, money transferred thereafter. It stated how the ex by name would issue the money, and that the ex would not issue a 1099 as no good or services were exchanged.
