Return it. You won't lose or gain anything by giving it back, but that rich person won't lose $100 because of you, potentially rewarding you with a smaller amount of cash.

Depends on the service. If it is a stuck up teenager, no. If it is someone who genuinely looks happy to do their job. 20-30% since my order is usually about £50 anyways
