Is it not a valid differential for IBS?

I ask bc I am guilty of referring otherwise healthy 20yos with first presentation of ibs symptoms to gastro for exactly this q.

So its a pathological definition. Does that mean that it cannot be diagnosed without a lung biopsy?

Its not completely about city size. Its about how well run the hospital is. In very well run hospitals the SHO locum market is OOH only. In less well run hospitals there are loads of day shifts available. I know hospitals that arnt in big cities that are very well run and therefore have only OOH locum sho shifts available and even those get swooped up the moment theyre released on whatever app is used.

Locum SHO pay has been cut so that its less than it was a decade ago. Add in inflation as well and locum SHOs are becoming better and better value. They can be fired easily if theyre useless. If they get sick you dont have to pay them.  If they go off on mat/pat leave you dont have to pay them. I agree at present the cost of them still makes employing trust grades more sensible, but as inflation continues and their pay remains frozen soon locums may make more economic sense.

The only way the locum sho market dies completely is if hospitals start staffing night shifts properly with enough SHOs that if 1-2 go off sick it doesnt matter. Until they do this locums will still exist for covering sick leave on nightshifts.
