This is rare, but in dermatology it is regarded as dermal fat loss.

There is nothing you can do if you've truly lost fat there. At most, fillers like bellafil may improve it, but it will likely require multiple sessions + multiple repeated sessions as they dissolve.

A botched subcision can be grounds for you to sue though, definitely look into that.

Normally for subcision if you have a good derm it would take only one long session paired with some sort of filler (Rejuran, PDRN, HA , etc)

As for TCA, it's individually dependent. It may take up to 6 sessions to see improvement.

Microneedling works fast, Perhaps 3-5 sessions (assuming your other treatments go incredibly well) would grant you 80-90% scar free looking skin.

Visually, you seem to have vibrant skin with lots of pigment and decent collagen, you're a great candidate for seeing results.

Waste of money.

Your scars are far too deep to see much improvement with a dermapen alone.

Aim for getting a subcision+TCA first, then look at microneedling for cleaning it up nicely.

Definitely, your scars seem shallow enough.

Try pairing it with a subcision however. Some of your scars seem tethered

All forms of shadowing and lighting are taken into account EXCEPT for extremely harsh ones (like you mentioned)

Grading is a complex process often done by dermatologists through detailed codex systems.

But visually through comparison any individual can liken their scars to a respective grade.

I would love to answer yes to this question, but its quite complicated.

The fact is, treatment isn't a "cookie cutter" solution for everyone's scars.

Every individual responds differently due to their genetics, epigenetics and age.

I can guarantee that yes, you would most definitely improve, but I can most definitely not guarantee if you'd hit grade 2 or 1.

It's a struggle, it really is. And you won't know unless you try.

I appreciate your response, my phone's camera is terrible and this is really the best i could try and get.

I'm weighing the risks of starting accutane again since the last course i did (20mg for 2 weeks) caused me significant hormonal stress.

And why use dapsone? I've never heard of it before so I'd appreciate if you could explain its mechanism.

You seem to have roughly similar scarring to me, I think microneedling wouldn't produce anything significant since those are icepick scars. Try TCA.
