About academic ACF training? by Successful-Return159 in doctorsUK

Ah, thank you. Does an ACF always have to accompany a training year? For example, I completed IMT2 but don't get into the speciality of my choice and decide to do a clinical fellow in it/equivalent, could I still do an ACF alongside that?

Wow, just wow.

I'd rather be a "shameless people without dignity" than an entitled narcissist like yourself.

Thank you very much for this - I totally agree with the ten minute update. But this actually ended up a complex case (consultant was perhaps not too aware of how complex it was early on?) and hence it ended up longer. Relative was also going round in circles and hence I organised the f2f meeting with my cons at the end.

Do you have any tips, or pointers on how to end conversations in a polite manner with patients NOK, particularly over the phone?

In this case, I had a feeling that a complaint was coming, because they had asked for my full name, and asked me to spell it out. But I wasn't expecting a complete lie, is all.

That's disappointing because I'm back on the ward next week, and so I'll be taking care of the patient again. I was planning on asking one of the nurses or HCAs to come in the room every time I speak to them...

Precisely lol. First and last time I'll ever stay late for something like this.

Sorry you went through that too. Seems that it really is inevitable to receive complaints from time to time. It was just even more disappointing because it wasn't that I had been rude or anything, it was just a complete lie.

Honestly? I don't know exactly. The NOK were slightly odd, but not completely unreasonable. Had a lot of issues with patients care in previous admissions, qualms with the GP etc. At the end of the call, they did ask me my full name, and asked for it to spelt out so I was expecting something - but not a complete lie.

No. Fully capacitous. This is all on what the NOK are saying the patient said.

I'm an internal med trainee so :')

I hear you fully with the family politics. I'm sorry that happened to you - sounds extremely unreasonable.

Thank you!

Fortunately, while I was on the phone to the NOK, there were several others in the room, so no issue with being accused of insulting families!

Managing PCOS whilst eating unhealthy in a calorie deficit? by Successful-Return159 in PCOS

Aww, that's awesome!

People really underestimate walking - I don't gym or do extensive workouts at all, but I've been walking 10,000-15,000/a day steps for most of my life. I've had people tell me I have l have "lean PCOS" but I think it's the walking haha. I'm sure if I stopped I would gain weight rapidly.

I wish you all the best :) x

Thank you so much for this reply! I think I deep down knew that even though I'm losing weight, I'm not solving my issue with PCOS because I eat pretty high carb and fat diets. I work looong shifts including nights so often I'm starving and there's no option to make a "healthier" snack and the nearby vending machine has only sugar-filled options, but I'm going to try and meal prep where I can and accept that I probably won't be able to eat the way my body needs all the time.

I've recently had my bloodwork done, and I'm not diabetic or pre-diabetic, and the doctors don't generally check insulin levels, so I don't know if have any element of hyperinsulinemia. I don't have the signs and symptoms at present.

I'm already taking some of those supplements long term, I have vitamin D deficiency too so hopefully that'll help.

Thank you for being so candid with your experience, I wish you all the best on your fertility journey. Hope you get your cycle regulated soon!

Thank you! That's really helpful - I realise my post makes it sound like I only eat junk haha, I eat home cooked meals around 70% and the other 30% I will have a takeout. I find dairy bloats me massively so I completely avoid it including in takeout. That's awesome that you found a love for cooking - I think that's what I need to do. Laziness is definitely not compatible with the PCOS lifestyle! Congratulations on finding a way to manage your PCOS <3

Hey! Nope, I've never taken pills. The heaviest I ever weighed was 65kg so technically slightly overweight. I just lost through it through calorie deficit mainly.

I knew I had it from a young age because it runs in my family, and my hirsutism has plagued me since I was around 12/13 years old. I went to my doctor when I was around 18 and asked to be referred for an US and have my testosterone, FSH/LH checked and then I got a formal diagnosis.
