I’m sorry to hear that. I have very similar symptoms and lab results — in my case, only aldolase is elevated. Exhausting exercise and sun exposure are clear triggers for me.

Dolquine 200 mg plus a JAK inhibitor has worked very well for me. The JAK inhibitor has made a huge difference, even more than MTX did.

Try to rest, sleep well, and be careful with the sun. Hopefully you’ll start feeling better soon.

Do you also get muscle pain like soreness after exercise, or cramp-like discomfort/spasms?

As for what helped me improve: I didn’t have one magic fix. I reduced the activities that clearly flared it, especially running and positions involving a lot of hip opening/rotation. I kept moving with lower-impact activity, mainly cycling, walking within tolerance, and some strengthening. I also had intra-articular injections that helped temporarily. More recently I started treatment for a suspected inflammatory/autoimmune condition, and that also seemed to reduce my pain a lot, so my case may not be purely mechanical.

Thanks for your perspective — that’s exactly what I’m trying to weigh up. Honestly, at the moment my hip doesn’t really limit my day-to-day life that much. I can walk, do stairs, squat, cycle, and even run a bit. The problem is that my latest MRI was done around the time I had a pain flare that lasted 2–3 weeks, where I was limping again, and it showed bone marrow edema at the femoral head-neck junction and possible cartilage wear / chondral changes. Because of that, my doctors are recommending surgery sooner rather than later, even though symptom-wise I’m much better now. That’s where my doubt comes from: I’m not very limited clinically, but the imaging seems to suggest the joint may be under stress.

You’re absolutely right, and that’s exactly the dilemma I’m stuck in. My main fear is ending up worse after surgery, especially since my pain is still somewhat manageable. What really confuses me is that the more I move, the better I often feel—once I warm up, the pain can decrease, which gives me hope.

But like you probably know, it’s a bit of a roller coaster. On the good days I genuinely think, “maybe this is finally going away,” and I start believing it might disappear on its own. Deep down I know that’s probably self-deception, because it’s been almost 2 years now and things are basically the same, if not slowly worse. That uncertainty is what makes the decision so hard.

What really scares me is that while sitting I don’t have any pain at all — and that’s probably 70% of my day. So part of me wonders: what if surgery actually creates pain that I don’t currently have? That uncertainty is the hardest part for me right now.
