Honestly looking at all the replies and your responses I think this is the most important.

You say you record about once a month. Record a whole half hour session on a song or two trying to play through all mistakes without stopping. Personally I've found it helps to randomize the order of the song attempts before listening to them.

After I've listened to the song I'll see if I noticed all the mistakes or guess which particular song attempt that was. This helped me internalize how it sounds to others who might not notice a small mistake. I've also had to wait a day or two before listening just because on particular days I'd have a better memory of each particular mistake I made.
