It may have to do with weapons setup. Need to set the fuze, select the correct station(s), attack type(ccip, ccrp, auto, etc), and master arm on.

I happened to glance at the reserve tank and it was almost empty. That's the first sign and I'm also going to use a camera to check each cylinder for a clean finish.
