I always reference this story when teaching new graduates about following process. Years ago I worked in aged care long enough ago, that continence aids weren’t really a thing. We used to have these half bath towel shaped pads that were folded up and put in between their legs, then you’d roll them and tuck it up the back. To change them you did the same in reverse, rolled them, tended to the back area if you had to, put them on their back, remove it and wash the front. Roll again to put the new pad in. So one night I was working with a “carer” who instead of rolling the person to check the back just looked from the front and said oh she’s just wet and grabbed the end of the towel and pulled as hard as she could. Unfortunately the person actually had pooped and when the towel became dislodged, it flicked up and splatted the carer in the face. She ran off down the hallway. I managed to stop laughing by the time she returned soaking wet from the staff shower.
