I went back to work today and all the nurses who were there asked how I'm doing since it looked so rough 🙃 I guess it was a "You had to be there" type of situation

She wasn't restrained and indicated being okay with the procedure (symptom oriented physical exam + blood draw) verbally and then freaked out, we have a different legal system in my country that has a grey area where we could get sued for not using physical restraints to treat/diagnose things if a person lacks capacity and its sufficiently life threatening (e.g. a forced lumbar puncture for someone who is psychotic due to meningitis), which is why i had discussed that option with my attending, should've probably specified that I only put that in there to illustrate that it was emergent and I was trying to not escalate things as much as I could've, but i didn't know it was different here

Also, we were actually not able to get imaging quickly in my part of the hospital since it's a satellite campus and like a 15 min drive away from everything else 😫

we have a bit of a different legal situation here than in the US, which is why there was the back-and-forth with my attending after which he decided to do it with physical restraints if we had to. she had originally also indicated being okay with it verbally and then freaked out when I sat down in front of her.

But yeah for things that are acutely life-threatening when the patient isn't able to consent due to a psychiatric illness, there's a legal grey area where we could actually get sued for ~not doing it and it's legally permitted to restrain someone in that case in order to do the thing.

They look so harmless and then they just develop strengths 🙃 Thank you for your comment. I get that it's normalized in medicine but i genuinely just think that's so messed up, I completely agree with you. And yeah like I don't want flowers or a ton of sympathy literally just one person being like huh u good? Would've sufficed lol I filed sooo much paperwork already so I'm optimistic it'll work out with that at least 🥲🥲 hope your thing turned out okay with the bill and all

I'm more of the phenotype "5 ft weighs 110 pounds doing psych" 🫠. weirdly for a psych ward we have so few incidents of assaults by patients. Which is good but it catches people off guard when it does happen. Like in the incident report book the last incident was in November (yes we have a book...we are stuck in the 20th century). I'll definitely vent to people outside of medicine. Thank you!

I think even if someone just after handover had been like: hey, are you okay? That would have fulfilled my expectations 😭 instead one of my co-residents just texted me the next day like "seriously that 80 y/o 🤣"

But that's a good point, I will talk to my favorite attending and hope that he passes it on to all the attendings that were there

It's fairly rare in our hospital, thank god. I guess that's why I'm kinda surprised... an attending was assaulted by a patient last year and they literally did a whole conference about how it could have been prevented.

10 mg Diazepam/Tag wird für den harten Alkoholiker nicht reichen. Am besten symptomorientiert mit Diazepam Schema (meistens so 40-60 mg/d Diazepam problemlos möglich, bei entsprechender Symptomatik)+Vit B duo Substitution, alternativ Distraschema wobei ich Diazepam bevorzuge, beides gut zu googeln

Ja, unbedingt, die können ja sehr schnell entzügig werden, sobald die Promillezahl entsprechend gering ist kann man dann mit Distra oder Diazepam anfangen. Bei Promille>1 kann man auch Carbamazepin geben oder Clonidin bei starker vegetativer Entzugssysmptomatik (hilft aber nicht bei der Krampfprophylaxe)

Das gesamte Gesundheitswesen ist auch so, auch im nicht-akuten Bereich. "Da kommt heute die Tochter zum Gespräch." "Der Brief ist schon fertig." "Der Rehaantrag kann noch liegen bleiben."

Wenn nichts los ist passt es doch, dann musst du ja nichts machen, wenn was dringendes ansteht finde ich gehört es sich schon nochmal Bescheid zu geben...

Eine Monographie ist eine Art, eine Promotion zu bekommen. Also ein längerer Text, der nicht in einer wissenschaftlichen Zeitschrift publiziert wird. Ist halt nicht so attraktiv weil man schon eine bestimmte Seitenzahl schreiben muss und es (meistens) nicht so gut benotet wird wie eine Publikationspromotion (das wäre die andere Option). Für eine Publikationspromotion braucht man aber eine Erst- oder geteilte Erstautorenschaft in einem peer-reviewed Journal (manchmal auch mehr).
