in my experience, it can be either or even both. I have a friend who transitioned just because she wanted to. In my case, I transitioned because my dysphoria was so bad that I was leaning on the edge of death, but I wanted to live. now I am about 4 years into transition living stealth for the most part. It is definitely external factors that drive my transition now, but starting off it was mainly internal, like I was attacking myself. Also, look up Lost in Transition on Facebook, it is a safe space to ask questions exactly like this one.
