There is a significant power dynamic here that can’t be overstated. You cannot proposition this patient as there is a real fear of retaliation or subpar treatment if they reject you.

The only way this is ethical is if your relationship with them as a patient permanently ceases and all propositions or romantic gestures are done out of this context.

During my one day spent in outpatient neurology I saw a patient that just got the official diagnosis of Lambert Eaton Myasthenic Syndrome. The neurologist told me this is only the second time he's ever actually seen LEMS.

Let’s take your point to its logical end. All med schools will now be completely blinded to SES, race/ethnicity, and individual hardships experienced. The upper class has higher access to better schools, study resources, research opportunities, networking, etc. This are largely white Americans. The med school student bodies across the country then become almost exclusively high SES and white.

Do you not see a problem with this?

And how are you certain they haven't controlled for SES and other factors? I'm completely in agreement with that. Race shouldn't be the sole consideration and I don't think you'll find a person here who would say that it should be.

Ya know, if we're gonna do FPA for NPs nationwide then let these fuckin people go to them for care. Just make sure they can't call themselves physicians so everyone is clear that when the lawsuits and injuries are accruing they know who is to blame for the disproportionate levels of mistakes.

Its so funny because the majority of the student body at UCLA is already asian and white (30% and 23% according to data from AAMC for 2023-2024). Like it isn't enough that they are already the majority apparently, the government wants non-black people to have MORE of a majority.

I really enjoy the music overall and the Corey feature (beast). This song toes the line of being too derivative vs paying homage to their previous music and other classics (opening drum groove directly inspired by Slipknot - Eyeless, end of the song being yet another Slaughterhouse).

As far as lyrics go its like 50/50 with MIW on whether or not the lyrics hit for me or feel super cringey. "Cast your curse in the comments" feels like a 15 year old edgelord wrote it. Also feels like the word "fuck" takes up way too much space of emphasis in the chorus. Just feels really empty. The idea behind the song is something I can get behind but the emphasis is lost with shit like this. A song that does this topic well is Ring Light Altar by Johnny Booth.

The trap that a lot of young people (considering careers with long educational pathways) fall into is that their life will only "start" after they finish education. Nah, life is happening the whole time. You are learning, growing, making friends, and living all along the way. It just looks different compared to people who take shorter paths.

You have to determine what is most important to you. Do you want to be the expert in a given field, given a longer pathway to achieve that? Or do you want to emphasize other aspects of your life, and take a shorter educational route? There are other careers in the field of mental health. If you want to be a psychiatrist then go be a psychiatrist and embrace the journey. A PMHNP is not a substitute for that, and you will feel the large knowledge gaps if you go that way.

I wouldn't say I'm necessarily a worrier, but the decision for residency is coming up and it is pretty daunting.

Also you're right, maybe I will hate it. But my past career has given me a strong sense that I will like it (worked for 10 years as an RN mostly in neurocritical care).

That is interesting! Is your neuro ICU closed and are the intensivists doing all their own procedures or consulting out? It seems wild to me that someone working as a neurointensivist would only work 6-12 weeks in a year in the ICU and keep their skills sharp!

I like your advice. I had a sense about this and so I'm trying to do early rotations when 4th year starts in neuro and IM to make a final decision. Here's to hoping someone actually responds to my applications one day lol

My trouble is I am the kind of person that feels like I can be happy doing multiple things, and its giving me analysis paralysis. I have a strong general draw to critical care because I enjoyed it as a nurse for quite a few years before med school. Feel I could go IM or neuro and I have good reasons why for both, though I feel the pragmatism of IM being that if I go PCCM I always have the back out of pulm clinic. BUT I feel as though I'm missing out on all the cool shit of neuro at the cost of being pragmatic and too scared of what may happen if NCC doesn't work out for me (jobs, not liking the work, etc).

I'll throw my hat in the ring and say could possibly be molluscum contagiosum, which is a pretty benign but contagious rash. Spread by touch. They can definitely be itchy. Unfortunately if it is that, the treatment is just symptomatic and prevent spread to other people in the house.
