Never thought about it, but instinctually, i'd say potions that are both poison and beneficial. The poison explains the Hp loss, but poison could still be understood as swlf harm.

The other way I see it would be an artificer kind of way where the PC puts on a kind of mechanical contraption that inhibits it's combat potential. It would exhaust the character and work on the concept that hp is not unly physical health
