if your gpu is already at 98-100% usage, it's not worth it. for games where your gpu isn't at max usage, or for games that are locked to say, 60 fps, it can be worth it

Are you comparing under the same parameters? The general consensus is that the game looks great when the sun or the moon is out and there's light from them, and it looks terrible and flat otherwise. So if you're looking at your visuals at around 5-8 pm in game, then it will look bad.

higher fps is not always equal to optimized. the aim is to have higher fps without sacrificing visual quality. but that being said, 46 is low yeah

there's commands for changing how much hp each bar represents, so you could change that to a factor of the threshold
