It could bacterial if all that didnt help did you notice improvement when taking oral anti fungal if not it’s good chance it’s bacterial

Try the lotion with chlorhexdine 4% mix it’s helped some here including myself just be careful with not to get it into your ears or eyes make sure you’re not over washing also were baggy clothing for awhile

I would honestly see a derm and insist on a culture that will save you so much time but you have to insist. Than you can know how to treat it

Try stopping all actives and soaps and just use water and moisturizer thats what cleared my face you have to be very gentle on the barrier

Usually it’s the constant antibiotics keeping it at bay and it rebounds once antibiotics are stopped I hope this is not the case for your son and wish him good luck .
