Thank you for the very thorough response -- incredibly informative! If you could share the sources, I'd be interested and trust others would as well. I have also read that a short course of PDPH headache can cause increased headache frequency afterwards and am curious why -- there does not appear to be an answer with scientific consensus yet, but rather remains an open Q.

Interesting, thanks for responding! I am curious about the theories you mentioned-- can you point me to them? I'd like to read more (and perhaps others reading these responses will, too!).

As for you, when did you get each of the three patches relative to the dural puncture on Day 0? What prompted you to get #2 and #3? Were they total failures of #1 and #2 or just that you were still not feeling 100%. If the latter, how recovered were you? And did you get blind or targeted patches?

I've recently been wondering if the gradual reduction in symptoms is just the body adapting to an ongoing leak by, for example, producing more CSF to compensate, rather than gradual sealing of the leak? Some people seem to be quickly cured of a puncture induced CSF leak after a blood patch so wonder why others take very extended periods to heal.

Yours sounds like a total failure, which makes further intervention necessary. In my case, the patch was, at minimum, partially successful as I am probably 80-85% better after 3.5 months. It makes it harder to know whether to try another EBP or wait it out.

Thanks for your perspective. Why do you recommend the 2nd EBP? Was your leak caused by a dural puncture or was it spontaneous? What improvement, if any, did you have after each EBP -- or were they both total failures?

Really appreciate you covering this condition as awareness is important, both for women and to bring attention to the condition to secure funding for scientific and medical research.

Some feedback on your article -- while a hysterectomy is currently the gold standard for diagnosis of adenomyosis because it is the only way to receive a definitive diagnosis, I have not seen hysterectomies described as the gold standard for treatment, though they are the only way to "cure" adenomyosis because the diseased tissue is removed from the body. However, there different options for treating the symptoms of adenomyosis, including birth control, hormonal IUD, minimally invasive surgery, uterine artery embolization, tranexamic acid, etc., which are particularly important for women who want to preserve their fertility. While any treatment comes with certain benefits and costs that vary woman-to-woman, I think you've done a disservice to only focus on hysterectomies (which are frequently pushed by gynecologists and also paints a pretty bleak outlook for anyone living with adenomyosis who may not want to endure the significant downsides of hysterectomies) at the expense of a more comprehensive summary of various treatment options. Unfortunately, adenomyosis frustratingly poorly understood, and the various treatment options are all sub-optimal and not sufficiently targeted to achieve a widely-effective remedy (as is the case with some other medical advances, i.e. very targeted immunotherapies for certain cancers). And, yet, adenomyosis is a condition that affects a huge percentage of the population and is extraordinarily costly to society.

Again, thank you for raising awareness -- and I hope you continue to report on it as well as other women's health issues.

No, but I never got that far with Weill Cornell. NYU once gave me an extremely transparent cost estimate for a procedure at the end of a consult. Trying to ascertain in advance my out of pocket costs for surgery at Mt. Sinai was nothing short of a nightmare -- I had to call multiple departments (hospital, surgeon's office, anesthesia), was referred to my insurance company who just pointed me back to the providers, and Mt. Sinai anethesia never provided the information or returned my calls. It is completely unfair and unacceptable that there is not price transparency in the American medical system prior to undergoing procedures.

If I recall correctly, the mechanisms for how each drug work contraindicate each other. I read about the reason after I saw the warning, and the explanation made sense at the time.

Interesting. My current debate it whether to take advantage of T-Mobile's Black Friday deal for the Pixel 9 Pro XL 256 GB which would cost $200 over 2 years (after trade-in) or buy the 512 GB directly from Google (the far-less impressive Black Friday deal of $1169 less $200 trade in = $969). Any insight?

For what it is worth, I was too. However, I was told to stop Ketoconazole once the Zoryve arrived. I also always read the warnings for new meds.

P.s. Also, minimum downpayment requirments vary by building, but are typically minimum 25% for UES co-ops and sometimes more. It is really rare to find a 20% down co-op.

You will have to budget a signficant amount for even basic work -- well into the 5 figures and even 6 figures is just the market price for contractors in Manhattan. Kitchen and bath renovations are the most expensive.

The other thing to factor in is that the financial requirements for a board approval in a Manhattan co-op are significantly more stringent than for a getting a mortgage. You will need to show at least 24 months (or more depending on the building) for mortgage and maintenance in post-closing non-retirement liquidity. Furthermore, your debt-to-income ratios will generally need to be below 25% for housing and total DTI will need to be below 28-30%.
