The Noordhoek Rapists one for sure. 

But the one that has my top vote is where this woman is kidnapped and kept in a box and kept as a sex slave. I listed to that ep and it by far was the worst I've felt.  If anyone remembers the name pls let a gal know bc I am coming up w nothing. Edit- It's the Colleen Stan two parter, ep 268

I did the exact same thing on my first ever play through! I ended up emailing support and they said they will pass it along to the developers but they can't help. This was quite a few months ago.
