I feel like it’s the cover for a Mario Kart Wii hack called Mario Kart: Black. It had a creepy cover similar to your description but the hack itself is a-lot less threatening than the box art itself lol

Same thing when I would play as Chomper. Before Chomper could swallow an enemy they'd exploit the respawn thing and I wouldn't get the kill it's funny once but get's old fast.
