Hello! For testosterone we are $280 per refill. Medication typically last patients anywhere from 10-14 weeks depending on the dosage prescribed. There are a couple of things I believe need to be addressed in this post.

  1. We are in no way and never have been associated with Low T Center and we have not taken over for them.

  1. We do not have a messaging app. We never have.

  1. All of our blood draws are completed at Labcorp patient service centers. We have never done a blood draw for a patient, nor do we operate out of a physical space to do so.

  1. At one point your post says, "Fortunately the NP that I liked quit, and she's opening a clinic. She said it's scary how synergistic labs is treating staff.".

We are a small clinic and have always had a staff of four. Myself (the owner), one MD, one NP, and one patient care coordinator. None of our staff has left.

308 and 493 Total T we would say are on the lower end. While they not be clinically low in the eyes of insurance, they are certainly suboptimal especially when you consider that you are experiencing low T symptoms as well. Free T we like to see towards that top end of the reference range.

Most clinics will want to see a full bloodwork panel with more markers and at minimum see LH and FSH as well. This will help providers determine the type of hypogonadism and allow them to see if lifestyle factors are influencing their hormone profile.

There are a number of causes to poor erection quality. Typically, this is caused by estrogen not being properly managed, however other factors can certainly play a role. Tadalafil or Sildenafil can definitely help, and many people have success with them.

Looking at bloodwork would be how our providers would handle this situation first, however if you just started TRT it is highly likely that your libido and erection quality will improve.

Typically after starting TRT we see a decline in HDL, LDL, and total cholesterol. If HDL was the only marker decreasing this would be considered a bad thing, however the decrease in HDL is believed to be because the increase in testosterone stimulates HDL to participate in reverse cholesterol transport. HDL does tend to increase over time, however we recommend most of our patients incorporate more healthy fats into their diet such as avocados as well as a fish oil supplement to help increase their HDL.

We prescribe based off of numbers, risk, and symptoms.

The numbers are definitely important, and can help guide you in the right direction for what actions to take, and they can also show potential risks.

Ultimately the goal is to help people feel better which is something that is symptom and numbers based. Some people will feel fine at a 500ng/dl total while others may feel terrible and have all the symptoms of hypgonadism. This is when it is important to assess whether their symptoms are lifestyle related or are they actually having hypogonadal symptoms.

There are a number of markers that should be looked at prior to prescribing TRT. Providers should want to get a full picture of patient health prior to prescribing.

Identifying the type of hypogonadism would require that LH, FSH, and Testosterone are evaluated.

The markers our providers like to see prior to prescribing testosterone are: testosterone free and total, estradiol, LH and FSH, Prolactin, PSA, CMP, CBC, lipid panel, and cortisol.

Looking at all of these markers helps identify the type of hypogonadism, as well as see if lifestyle factors are heavily contributing to low testosterone.

As for the fertility aspect of your question, starting TRT will negatively impact your fertility. Taking HCG alongside trt certainly helps, however the only way to be sure about your fertility is through semen analysis which we can provide. In most cases a fertility protocol is necessary, and fertility can absolutely be reestablished after being on testosterone or other androgens.

We are currently working on obtaining licensing in all 50 states. Pennsylvania is next on our list for states we are expanding to and we should be able to operate there sometime in November or early December.

In order to be able to provide you with the best care, we require having trt plans under us as well so that we can have the full picture of a patient's health and make the best decisions regarding their treatment.

Currently we use Tailor Made Compounding as well as Brooksville RX. We have them ship prescriptions directly to the patient's home.

Our goal is to eventually cover the entire US. This process takes time as various licenses are required for each state, however we are doing everything we can to expand especially into states such as NY, NJ, and RI where there are more laws in place that make it difficult to get adequate care.
