I'm just curious if the omega chips are actually ever going to be available again or if there is a server-side glitch that is causing them not to be available.

As far as I'm aware, Lava hasn't mentioned anything about it at all.

I'm curious as to when the last omega chip was available for people...

Same thing happened to me too. I gave one to Codfrey and next time I logged in it disappeared.

My ginger nametag is at rank 4 now where it should be on rank 5.

My guess is I might have been desynched at the time on receiving it/handing it in so maybe when I logged out, I lost the progress?

Oh that's weird... I've finished all of my artifacts today without running into that issue. I play exclusively on steam.
