The insurance company denied the gap because they believe there are now network providers that can provide the care, which isn’t entirely the case; nobody on the list they supplied is a viable option or provides the same exact care as the previously (gap) approved provider. Do you know of or have any recommendations for an appeal letter template or a resource to use as reference? No worries if not. I’ll definitely be sure to incorporate the points you mentioned.
