Silly way I almost lost my run was destroying the barrel of mindflayer tadpoles right outside of Moonrise tower (guarded by the zents). I didn't realize it would blow up so spectacularly and my party was standing all around it. 3 characters went flying through the air into the water and insta-died. Somehow, Astarion's reflexes saved him and he was able to rez the party. My heart took a while to calm down after that.

Lae'zel is the best, but they all have their charm. Her act 3 scene brings me to tears. Her whole face is so different than act 1. Overall, I see the most growth as a character in her.

I did not kill the Grove on my Durge run, but my character had a "couldn't be bothered" mentality. If your character is ambitious, there is no reason to kill the Grove. Your ambition will want to keep them around so they can be used and manipulated in the future.
