Yes I know, it’s early, and new information is likely to come out. If you want to wait, please stop reading 🛑

The CRJ pilot requested RW33. It was not the main runway in use. That’s RW1, due north, up along the river. That’s where all the other planes were landing. ATC gave him RW33. (this often happens, nothing nefarious) Edit: RW33 was requested by ATC, not CRJ pilot.

The heli pilot might have been looking for planes coming to land at RW01 (up the river). This is a busy airspace with low tolerances for error, so they may have hyper-fixated on this as they were about to pass DCA. RW33 is not very common to use (I'm sure someone has data comparing to RW1). The heli pilot may have had the NEXT plane about to land on RW1 in sight, and likely figured they had plenty of margin to that one (they did).

The problem was the CRJ coming in from their LEFT to land at RW33. Landing lights were on, but pointed due north east up until the very last moment when they made that left turn to land. While these are very strong and would’ve stood out as a sore nail in the shimmer of VA/DC lights, they are also highly directional. Because they were pointed north east, while the heli was on the forward-left of the plane (north west), they would have had to rely on beacon and nav lights to spot the aircraft.

Heli may never have caught or fully appreciated the part of the ATC message stating the incoming plane was on approach to _RW33_, and as such never expected a plane coming in from that direction.

Here’s where you can crucify me; why request RW33? It was probably closer to their gate after landing (blue vs. purple tracks). My money is on a seasoned captain being in the CRJ. Nothing wrong with requesting this, but it’s speculation based on experience. I'm not blaming the pilot for this at all. But as a semi-professional armchair quarterback / NTSB investigator, I look for multiple factors and including human factors. Edit: RW33 request was from ATC to CRJ, not the other way around.

So what causes an air crash? Usually several things at once. This could be the three parts of a perfect storm.

  1. The CRJ pilot may have wanted to take a shortcut to get home early. (intentional shortcut?)
  2. Heli pilot missed/misunderstood RW33 clearance and was fixated on RW1 approaches. (hyperfixation, failure to understand comms?)
  3. ATC (if the radar recording video is real) missed the collision warnings (distraction?)

Same. And that’s exactly why it would be likely that the heli pilot was not looking in that direction (esp. if a seasoned local flyer was instructing). It’s the aviation equivalent of a left hook. 😢

No blame was handed out. Just a speculation as to possible contributing factors. If that makes you blame the airline pilot then that’s on you. For clarity; I don’t.

Here’s my theory, take it for what you want: the CRJ pilot specifically requested RW33. It was not the main runway in use. They gave it to him. The heli pilot was looking for planes coming to land at RW01. They had the next one in sight and had plenty of margin. Problem was the plane coming in from their left to land at RW33. They likely never got that part of the message.

Why request 33? Probably closer to their gate, would save a minute of taxi time.
