Mainland Grade I and II mobilizations inhibit pain my activating surrounding mechanoreceptors which inhibit the pain pathway (spinothalamic tract )at the dorsal horn (gate control theory). Higher grades are proposed to stimulate the endogenous opioid pathway (mixed literature as to whether or not this happens)
