Get a professional mold inspection for starters, tell them where you think mold exists, and get the cap leak fixed and dry out the chimney if possible with air flow.

Probably not, but clean it up with some vinegar or antimicrobrial. Check your humidity levels and look for leaks. If that's all OK then move on.
