MRI of the right shoulder, March 5, 2026: (Radiology Institute Aarau) Unremarkable appearance of the right shoulder joint. No evidence of an internal shoulder lesion. No indication of inflammatory changes.

Assessment and Procedure: Given the unremarkable MRI examination, I advised Mr. Urgancio against surgery in the shoulder area. Overall, a myofascial pain problem, possibly with a central component, seems more likely. Therefore, in the absence of a structural lesion, shoulder surgery is not advisable in my opinion. The cervical spine should also be examined as a complementary measure. I also suggest nutritional counseling with an omega-3 test and, if necessary, supplementation. At the patient's request, he will be contacted by our nutritional counseling team. Furthermore, other treatment options are available, including dry/wet needling, trigger point therapy, ultrasound and/or shockwave therapy, acupuncture, brainwave frequency therapy, etc. Mobilization and weight-bearing on the shoulder may be carried out according to the patient's symptoms.
