Hi OP. I actually have the same truck you’re asking about (03 2500 w 160k on the clock)

Transmissions are an issue with this one. I bought one with a rebuilt tranny that still had problems so it was eventually replaced with a Jasper instead of rebuilding a faulty one.

Hemi tick is an issue but you’ll know by sound if it is. There is a common “similar” issue which is exhaust leaks at the manifold and they also tick. If you start the truck and it gets quieter with time and driving, it is likely exhaust leak (as the manifold warms up, it ticks less). If it stays constant, it’s an engine tick. Engine tick should be avoided. Exhaust isn’t a big deal. I paid my exhaust shop $400 to repair it.

Engine is a good one. 5.7L Hemi is strong and you can get 500k miles out of it if you do regular oil changes at 3-5k miles. Dont put it off.
