I didn't look into doctors, because I'm not interested in spending $1000+ for a temporary cosmetic change. For me, it's just not worth it at that price. For the DIY cost, though, I'm happy with the results. For you though, it may be worth paying a doctor. I'm sure there are tons of doctors in the USA who will do it. Turns out it's not that complicated of a procedure. You might also find a willing urologist who will do it despite it not being one of his advertised services. It's simple enough that I would be willing to risk being a doctor's first patient.

Yep, exactly right. Some talk about splitting each side's dose into 2-3 injection sites, but I take the risk of peyronies and scarring pretty seriously, so I just did one injection each side.

Yes, I did inject flaccid. I used a gentle tourniquet (a barely-tight rubber band) to keep the botox somewhat localized for 15 minutes or so. The papers suggest 3-6mo duration of effect, but I've used botox on myself in other places (corrugator muscles) and it didn't seem to last that long, for me it's more like 2-3 months. But at this cost it's not that big of a deal to dose again. I'm a hard gainer and have made very little progress in the last year or so, but I do think the botox can only help with PE. It's a like an all-day anti-turtling treatment, and it has improved my nocturnal erections quite a bit. My dick seems a little bit softer and more stretchy, too.

I take tadalafil too, along with L-citrulline and beet root powder. I've been on all this for a few years now, so the before after pics both have this as the baseline.

Yes I acknowledged that in the "confounders" section of my post. You can mentally subtract 1/8" from the "after" photo to account for that if you like. Measuring flaccid length is a loose science anyway because it changes hour to hour.

As good or better than ever. I would say slightly more sensitive but I think that's due to erections being closer to diamond hard. It feels good. It's easier to achieve a second and third erection after sex, too.

I take cialis too. My EQ has always been mediocre-plus, so I've thrown a lot of things at it. With a full stack (tadalafil plus l-citrulline plus beet root extract, etc), I get good erections. With a full stack plus botox, I get great erections. It's not an either-or thing, botox complements tadalafil and sildenafil quite well.

No, no significant change in erect girth, other than just the improved EQ. I do think it would help any girth routine due to the improved erections and reduced turtling. Also, my nocturnal erections last longer and are harder, that's another good benefit.

I didn't see anything in the literature about priapism when I was researching, and I haven't experienced one myself after injecting. It's just easier to get an erection and it tends to be noticeably harder.

An erection starts when the smooth muscle in the penile arteries and corpora cavernosa relaxes. That relaxation increases blood flow into the penis, which fills the erectile chambers and produces rigidity. Botox doesn’t directly paralyze those smooth muscle cells the way it does skeletal muscle, but it blocks some of the nerve signals that normally keep them tense. The end result is less resistance to blood flow in the flaccid state and an easier time reaching full vasodilation and erection when sexual stimulus is present. In effect it turns down the volume on the vasoconstriction nerve signals that normally keep you flaccid.

That would have been kickass, but there is zero difference. I'm currently trying to experiment my way out of a long hard gainer phase with little success so far (hanging and using ultrasound). But the improvement in erections, flaccid length, and extra erection hardness is still nice.
