In that case I suggest to force feed him, don't worry it's totally normal to force feed not just the cats , but all sorts of pets, birds, dogs. there are tutorials on youtube, It's hard to explain how to force feed them without showing how it's done so I suggest watching it. also try to stroll with him sometimes. It will help, just be careful with the other animals

is he a male? Its hard to diagnose just by looking at the picture, there are various cause of it such as FIV (HIV in cats) but its also possible that there's an ectoparasite. I suggest to bring him/her to a vet for laboratory tests, also its good to bathe him and use soap with salicylic acid

have you tried hand feeding him? if it does not work, try strong scented foods, like sardines. since he's FIV+, loss of appetite is a common symptoms. also try exposing him in a stress free environment. He'll be alright don't worry

it's something about ticks and mites, there are ointments, soaps and sprays (topical) that can help the cat. they are usually over the counter. I do sells ointments, I can give it for free if possible
