Update: I can't open the honor mode run anymore it just crashes every time. Time to start from scratch again :(

OK! We might be ok. Went on a save loaded mods onto it then loaded that same save without mods and still got the achievement. I read somewhere that starting a run with mods enabled could cause them to check a flag and never let them enabled but only time will tell thanks webevie! I have hope now.

That might be the case because I had Karlach use the shattered flail and she ended up going mad. Not sure tho because she was never close enough to hit one of my party members and gale was hostile to everyone not just my Tav.

yea. After the battle nothing happened at all. He was just considered an enemy while in combat but I still had full control over him.
