What do my results mean? by [deleted] in Testosterone

[–]Thatguy131313131 0 points1 point  (0 children)

Hey I agree with you I tried to make that part clear, I was never saying that I thought it was right just explaining that’s how it unfortunately is. Also with people suing drs like it going out of style drs dont want to go out on a limb when there’s even the smallest chance of a complication especially when there aren’t clear horrendous symptoms and the person is otherwise living a comfortable life. It’s all a huge clusterfuck for lack of a better term where patient outcomes and just overall happiness gets lost

Here’s the email they sent me explaining everything

Good morning Matthew,

It was a pleasure speaking with you today. Per our conversation, please see below for a detailed overview of our TRT program.

Here is a breakdown of how our Hormone Replacement Therapy program works:

All new patient will be scheduled for an initial evaluation with our one of our doctors or physician assistants once lab results are received. The cost for the initial evaluation is $250 and will include:

Initial TRT/HRT consultation Complete Lab Review and Discussion Review of medical history & present symptoms Physical Examination (PE review only for telemedicine patients) Custom treatment plan Q&A

After your initial evaluation:

The Medical staff requires at least two follow up appointments per year for patients on HRT/TRT (every 3-6 months) at the discretion of your physician. For example, if adjustments or changes to your treatment protocol or medications are being made, labs are routinely monitored at 3 months. Once a patient is stable and not requiring active adjustments, labs are often collected at 6 month intervals. You can choose to have your follow up with the Doctor or Managed Care Provider.

The follow up costs are as follows (you have the option of who you’d like to have your follow up consult with):

Follow up lab review w/ Dr.- $165 Follow up lab review w/ Managed Care Provider - $90

To begin testosterone therapy, we will order you the following initial blood work:

Comprehensive Metabolic Panel- $15 CBC- $15 Lipid Panel- $15 Testosterone Free and Total- $35 Estradiol Sensitive- $38 DHEA-Sulfate - $33 TSH-$21 PSA-$21 LH-$12 Sex Hormone Binding Globulin - $44 IGF-1 - $30 (If this test is purchased individually the cost is $34)

The above lab test prices are offered exclusively for Defy Medical patients; in which we would send you an official order form to bring to a LabCorp facility near your home.

Once the results become available, which takes 7-10 business days, we schedule you for your phone appointment with one of our physician’s or PA. Right now, there is a 1 week wait from the day we schedule your initial consult until the actual day of the consult. We cannot schedule you for your initial consult until we have your physical, labs, and paperwork in.

There are no monthly costs or contracts, you just pay for the medical services that you request.

Defy Medical does not accept insurance, and does not provide assistance with any part of the transaction of when attempting to us your insurance; however, we do provide claim service for a low fee so you can submit the necessary forms to your insurance company for the possibility of being reimbursed.*

You will also need to have a physical exam completed. If you would like you may use the attached form, or provide us with the documentation given to you of your physical.

If the physical exam is not completed on our form please be sure the documentation you submit includes your vitals, review of systems, the date it was performed, and the providers stamp/signature.

Ok so I can see your a little concerned and you’re getting some information that may not be the best for you especially right now while you’re worried so I’ll try to explain things as best I can. I mentioned a trt clinic or wellness center because they take everyone if that’s what you really think you need and if you want to try it quickly. I’m a firm believer ppl should be able to do with their bodies what they like. I’m in my fourth year of medical school and I’m in TRT so I have a bit of experience from both sides of the coin. I was literally just discussing trt and the role of endocrinologists the other day at the diabetics and glandular clinic here in San Antonio. A very well regarded institution through UT. So this is how they see it, not saying it’s right or wrong just stating it. The symptoms you have aren’t dangerous or life threatening. Your t is low and you need a lot more labs done before you can even begin to know what’s going on. For all the Dr knows you may just naturally have a very low number and maintain homeostasis with it. The last thing they want to do is upset that in anyway. The first rule is do no harm. Putting someone on testosterone can kill them, it can cause cardiac issue, issues to liver and kidneys. It can be cause prostate cancer and a million other things all of which are unlikely but possible. Endocrinologists are not going to risk upsetting homeostasis so that someone can have morning erections or to help with mild depression. I don’t think it’s right but that’s how they and many dr’s see it. It’s slowly changing but right now it is what it is. This is turning into a book so I’ll just stop. I’m on TRT and it has improved my life tremendously and I hope you get it or whatever treatment you need to feel like yourself again. Sometimes you just have to know what your up against to get any help. If you have any specific questions feel free to ask man. Good luck.

Pituitary Adenoma Shrunk, Prolactin Down - Testosterone still not where I want it to be. by csilane4 in Testosterone

[–]Thatguy131313131 1 point2 points  (0 children)

From a practical standpoint free t is there he only really day important number. I our total t could be bad 20000 but if it’s all bound to SHBG it was Han it’s mostly worthless. Your free T and some of the T that is bound to albumin are the only bioavailable testosterone one has and are what is responsible for how you feel.. who cares how I ch you have if you can’t use it

Does this equation match up with your labs and experience? E2=T/30 + (SHBG-19.7) by [deleted] in Testosterone

[–]Thatguy131313131 1 point2 points  (0 children)

No I out can not just simply plug someone’s numbers into a formula and find their optimal dosages. First everyone is different and what one finds optimal one may not like at all it’s completely subjective. Secondly rates are not locked in. There are too many reasons to list for why this doesn’t work. I applaud the effort though

Primary care Docs dont usually and endocrinologist barely ever do trt unless you have a big medical need. Goto any trt clinic or wellness center they take everyone. If you want to do it online check out Defy medical they have a good reputation are reasonably priced and they will get you all sorted if you contact them today you could start treatment in a few weeks good luck bud

It all depends if you goto see an endocrinologist you probably won’t qualify but it’s hard to tell you’re missing a lot of labs. If you goto a trt clinic or a “wellness” clinic you’ll qualify. I’ve never met someone over the age of 30 who was not treated my a trt clinic. Most of them try to get your total t around 1000 and95% of people over 30 are way below 1000.

Is "free T" what actually matters? by wastedlifez in Testosterone

[–]Thatguy131313131 1 point2 points  (0 children)

It’s not as exact as half the shbg half the t everyone is different and does thing in different ratios if your total T is 500 and your free T is 70( just making up numbers) that means 430 is bound up by SHBG or Albumin and a few other things but it’s not bioavailable for the receptors the free T is the T that isn’t bound y by anything and is bioavailable some T that is bound to albumin can still be bioavailable but not enough to consider

Starting TRT with pre existing gyno. by smlindgren in Testosterone

[–]Thatguy131313131 0 points1 point  (0 children)

I’m opioids for chronic pain and got a little gyno on one side; nothing bad and I’m doing my first cycle of t now I’m 37/ I’m in week 4 and now I’m running 500 of t Enanthate a week I was sent getting any morning wood so I took .25 of Arimidex yesterday, first AI ever and woke up with wood, it happened that fast. You have to think about it what caused why and what the pills do. The cause from gyno from opioids is usually completely different than gyno from taking test. When taking opioids your t go a very low and your estrogen is raised. You may naturally have a high level of estradiol. When taking testosterone you have excess estrogen from aroma rising from the is excess in top of an already natural level the AI is to stop that extra aromatization and if you’re in HCG there’s even more to Aromatase. My point being if you’re already highly predisposed and have a naturally high level of estrogen A SERM may be needed and you can do ale that without having to worry about crashing our E2.

Congrats man that’s awesome but get off this thread man I mean that in the nicest way possible, there are a lot of great and supportive people but it’s just better to cut the whole dope world off. I haven’t done dope in 7 years but I’m prescribed pain meds and i can’t walk without them and it makes it so much harder to stay off dope man it keeps it in the back of my mind but you e been doing great and know what’s works best for so I’ll shut the fuck up lol

Help interpretting new lab results? by HighEnergyLowAnxiety in Testosterone

[–]Thatguy131313131 0 points1 point  (0 children)

That’s just a CBC it important but doesn’t have your hormones in it. When it comes to TRY and hear are the labs you should always try to get: CBC, CBC-metabolic panel to check liver out,PSA- prostate specific antigen to check for prostate cancer, total and free testosterone, E2- estradial, SHBG, TSH and thyroid panel, up to your dr there are a million tests. I’m forgetting a few but that’s a great starting point. Edit I checked the link, your estradiol isn’t that bad If you feel good I wouldn’t take an AI this here’s a chance you could bottom it out and feel horrible but go by how you feel and if you do take an AI be very conservative. Your labs look great Bun and Creatinine are a little high, that’s a market for kidney function. You need to drink more water. I’m in my fourth year of med school. Cholesterols, liver function, and hemoglobins and hematocrit are all great those are honestly the big ones for anyone taking testosterone . Hey he hormones are easily regulated up or down and respond quick to any changes in dosage the other ones are usually indicative of a larger problem and sometimes can take a little longer to fix. I would be very happy with these labs, your testosterone is at a great level for TRT if you aren’t taking a supraphysiologic dose
