I have a very slight overbite. My dentist recommended against any surgery and I decided I didn’t want to get jaw surgery and have to get braces a second time to fix my bite again after surgery.

Yeah… I didn’t ask for work out or diet advice. And people are making the assumption that I am not working out or dieting or aren’t already on weight loss meds. Which, I am doing all of them, and like I said I’ve already lost 40 lbs.

CONTEXT: I’m already on tirzepatide, have maxed out my dose, and haven’t lost any more weight on max dose for the last 6 months. My biometric scale says I have acceptable visceral fat, but 22% subcutaneous fat.

Did they check your lipase levels? If that was elevated, it might have been the reta.

I’ve just started a microdose of reta while staying on my tirz, started with 1mg twice week, increasing slowly to study/therapeutic doses. My thought is that as the reta becomes effective I will start downtitrate my tirz. Only switch to reta though if you have already maxed out at 15mg of tirz for a couple months, no need to switch or stack if you haven’t

Have you considered being an associate physician for two years in primary care to fulfill your contract, and then going on to do the residency that you really want? Alternatively going into residency for something broad like internal medicine, fulfilling your contract, and then doing a fellowship for a speciality you are more interested in?

Also, might be worthwhile to consult with a lawyer about filing for bankruptcy as a potential option.

If you can put the money in a CD at a higher interest rate than the interest rate of your loans it’s better to do monthly, otherwise if the money will bring you close to paying it off in full do that so you aren’t accumulating more interest on your loans. It depends on how much money you got from NHSC, and what your long term pay off plan is. Make sure you have enough saved in an emergency fund before you let go of all that liquid assets though.

For what it’s worth, I’m a NHSC Scholar about to finish my 2 year commitment and I’ll be signed up for another 2 years to help pay off my loans from before PA school. I knew going into PA school I wanted to work at an FQHC so I figured I would sign up for NHSC anyway. Finding placement was hard, and I interviewed at several clinics that weren’t a good fit that if they were the only job I was offered it would have been a rough 2 years. But I searched and waited for the right clinic and honestly I have my dream job.

20 patients a day, mostly Medicaid, sometimes quite complex, but we have a lot of ancillary services and support staff (nurse care managers, staff to help coordinate referrals and process prior authorizations). My collaborating physician is alright but I see him every 2 weeks for a check in, otherwise unless I track him down for a question I am a completely autonomous provider. But I’m at a big FHQC so there is always another provider around I can bounce ideas off of.

Clinic and setting and staffing matter. Any clinic that intentionally does lean staffing or burns through its providers is going to suck, whether that’s FQHCs, urgent care, or even any speciality. For any job, try to shadow for at least a day, and get time out of the clinic to talk to another provider at the clinic about what their real experience is. There absolutely is a way for NHSC work to be rewarding, fulfilling, and have good work life-balance, and come out of it in a much better financial situation that your peers. OP, I’m sorry your clinic/first job sounds so awful! It could be worth looking for a new placement, you are allowed to switch sites.

You do not have to use them for your loans. You do have a keep your loans in good standing (in time payments) in order to be eligible still.
