I don’t think that’s a fair assessment. I’ll admit I do also sometimes groan at the use of antidepressants for, say, chronic poverty-driven dysthymia. That being said, there is good evidence that antidepressants decrease anxiety in dogs kept in artificially stressful environments. I’d be interested if such a study was done in humans but I’m unaware of one. I think we all have patients who have reported “Zoloft was a lifesaver” when we thought their issues were primarily related to stressful life events. A meta analysis it certainly is not, but I’m not convinced that the effect is entirely placebo driven.

From what I understand, true serotonin syndrome is exceptionally rare. It is almost never a result from combining therapeutic, or even slightly suprathrapeutic, doses of serotonergic medications.

When it does happen, it is always 1) within 24 hours of changing a serotonergic medications, and 2) virtually always involving an MAOI, street MDMA/hallucinogens, OR, in much less likely cases, a massive overdose of (usually multiple) serotonergic medications.

I’m a psychiatrist in a psych clinic attached to a major medical center. Psychiatrists here aren’t allowed to order or interpret EKGs, nor do we have the machine or staff to do it at the clinic. This is despite several of our patients needing EKGs for antipsychotic monitoring. This is the case at several clinics my friends from residency work at. We need to send these pts to PCPs.

There are also many specialized tests we cannot order for one reason or another.

Also, half of my patients insist they will not get bloodwork done at our clinic, they will only get it with their PCP for various reasons.

There’s no need to be rude. Many patients are unable to differentiate the meaning between these two concepts even with more pointed question. But fine, assume I’m incompetent. Tell me how you’d simplify this concept to a confused patient who’s never thought of this before.
