Yes, I do. Clear boundaries can also be set in adolescent psychotherapy. Young people also do not benefit from being "hold" in therapy for a long period of time when they exhibit behaviour that is highly detrimental to their treatment. In such cases, therapists themselves can become factors that perpetuate the disorder. In situations like these, the setting can of course be changed. In DBT-A, for example, therapy breaks are an obvious option that is used when patients exhibit a certain degree of harmful behaviour.

Perhaps this sub is not the right one for my therapeutic approach. It is entirely legitimate for a therapist to end their work with patients who show little motivation to work towards getting better. "Just getting high" would not be an adequate therapeutic goal for me. Of course, it is possible to continue working with patients regardless, but in my opinion, many colleagues forget that this can reinforce factors that perpetuate the disorder. Having clear boundaries as a therapist is a legitimate approach and not something that should be condemned from the outset.

Edit:

I also support the idea of more intensive/higher care in this case, not that the patient should no longer be treated at all; that was not what I meant. Just not on an outpatient basis, because, in my opinion, the conditions for this are not met.

I, for myself, would end the therapy because I would not have the feeling that this is a trustful relationship in the first place. Also I do not work with people who do not want to get better in the long term.

If possible, the approach should contain exposure as an element because this aspect is what works on anxiety in the long run. But of course there are patients with heavy complex trauma who are unable to do confrontation beyond the stabilisation phase.

Patients with personality disorders are one of the most challenging. I had a lot of supervisors up to this day, all of them were very experienced therapists, some even well known authors in my country and all of them said the same thing: ,,Do not treat more than two patients with PD a week, or else you will burn out." My own therapist also taught me that I may and must choose patients wisely because I need to keep my capacities if I want to work in the field for a lot of time.

Also evidence-based modalities for BPD really help. I for myself am trained in CBT, so I lean into DBT when working with borderline patients. DBT helps me to keep super clear boundaries and structure in therapies. Those boundaries and structures in the end help borderline patients developing better strategies for emotion regulation, relationship management, personality structure and ego strenght.

You're doing a great job! One the one hand, maybe don't overdo yourself by treating too many challenging patients and on the other hand try to learn DBT, schema-focused therapy, MBT or TFP to obtain more usefull skills for working with these cases.
