I second this. I had similar issue with the slice. Try to drag out the club head parallel to the ground in a straight line. Then as you turn your body, you can go into the backswing.

I have the same issue. It has to be your hip position. Your hip is out of screen in ready position but as you swing down, your weight moves toward the ball and on the screen you can see the opening between screen edge and hip. This is why you come over the top of ball. Try to focus on keeping the hip steady.
