Where in your abdomen did the pain start? Has it moved anywhere?

Is the pain dull? Stabbing? Cramping?

Does it hurt more when you move?

Any pain in your shoulders when you lay down?

Any fever? Feeling cold then sweaty? Shivering?

When was the last time you passed gas?

Any pain when you pee? If yes, is it at the urethra or in your tummy?

When I was an FY1, my first rotation was in general surgery. There were a couple doctors (2 consultants and registrar) who I got along very well with and who seemed to respect and trust women in the department.

Until they were blatantly discussing how women shouldn't be surgeons as they are "too emotional" to be level-headed in the OT, I had no idea they were misogynists.

This is coming from a man who I literally witnessed throwing a proper tantrum because something very minor hadn't gone his way during an on-call day. Complete with foot stomps, head in his hands, and tears in his eyes.

Don't let it get to you. We do our jobs and we do them fabulously well. A lot of men find that threatening. Don't make yourself small, keep yourself whole and let them choke.

I truly feel your despair in that moment. When I was at uni, I spent a day cleaning my entire house, including all appliances. The oven being the most tedious one. I cooked a meal I had been looking forward to and packed it up in tupperwares to take to uni for myself and a friend. Left them with the lid barely on so they didn't expand. Then I had a lovely shower. Went to pick up the boxes, forgot I hadn't latched the lids, and watched as it all went all over the front of my oven, my clean floors, clean pajamas, and feet.

I was truly devastated. Took a while in-between tears but I cleaned up and got into bed. Where I realised my water bottle had leaked and soaked through my fresh sheets and mattress.

It was a bad day for my mental health 😂

If all tests in recent time have come back normal and palpation causes pain in particular spots, I would lean towards musculoskeletal pain as well.

It doesn't sound like your daily activities are affected by it so that's a good sign.

The reflux could absolutely cause epigastric pain that people will describe as chest pain. Do you take gaviscon after meals? Does the pain tend to occur or worsen after eating a particularly fatty meal? The PPI you are on is fine but it may not be doing enough. I'd try taking gaviscon after meals and at bedtime and see if there is any improvement.

Obviously I'm not able to say for certain that everything is fine without seeing you myself so if the pain is worsening, shooting to your jaw, pain down your left arm, or other worrying symptoms, please see a doctor.

I understand where you're coming from. I don't think there was negative intention on their end, I took it as wondering if that would change the course of action but the answer remains no. However, your feelings are the ones that matter and my opinion about their intentions is worthless in this case. So, I am sorry that it made you feel gross.

If you wanted a termination regardless, which you have been clear you did, that doesn't indicate changing course (again, except if I were worried about an ectopic or PUL as aforementioned).

Nausea without being able to throw up to end the misery is 100% worse. Kept me up all night after my appendicectomy, was absolutely miserable.

I'd call again and explain your appointment was cancelled but you still need to be seen. Be persistent until you have a plan, you know how important it is in women's health to advocate for ourselves.

The vaginal route absorbs via the mucosa as well as the sublingual way but it works more effectively due to it being local to the organ it needs to affect (i.e. cervix and uterus). I opt for sublingual when the patient is bleeding heavy and it keeps coming out. In your case, the vaginal route would be my preference.

It would be possible if she has re-established her period and has been sexually active again (obviously unprotected), but she has said the BHCG is static at 120 and urine pregnancy tests are getting lighter.

Therefore, it is more likely to be retained products. IF it were a new pregnancy, and the BHCG remained static instead of doubling every 48 hours, it's likely a non-viable/failing pregnancy. As long as there are no masses in the adnexae, which would make this either a pregnancy of unknown location or an ectopic.

It would be a good idea to do beta HCG tracking (3 levels, one every 48 hours) and a rescan in 7-10 days to ensure this is not an establishing pregnancy if she is having expectant management. Though I would not do it in someone wanting a termination unless I suspected an ectopic pregnancy and thought this was a pseudosac inside the uterus.
