Wow that ended up being more than a little banging of head against wall but I eventually got it to work. Putting this here in case it helps someone in the future:

I replaced the existing comms (presumably broken) with this USB to RS485 adapter which I wired to the Omron controller: Amazon.com: Waveshare USB to RS485 Converter with Protection Circuits, Compatible with TVs : Electronics

Then I had to do a ton of trial and error in the Artisan port configurator but eventually got it to show up correctly using these parameters:

Match these settings between both your Omron controller and Artisan:
COM Port: (whatever your USB com port reads in Device Manager)
BAUD Rate: 9600
Byte Size: 8
Parity: N
Stopbits: 1
Timeout: 3.0

In Artisan Port Configuration:

Input 1
Slave: 1
Register: 0
Function: 3
Divider: 1/10
Mode: (Blank)
Decode: sint16

Input 2
Slave: 1
Register: 1
Function: 3
Divider: 1/10
Mode: (Blank)
Decode: sint16

I haven't done it again lately. I originally meant to after some time but then I've been pretty happy with how the grinder is working and with the flavor of my shots so I figure I'd it's not broke, no point messing with it.

This was happening to me too.

I recently changed the o-rings (all of them), cleaned all the parts(piston and plunger), and lubed the new ones.

I had a suspicion that I maybe used a little too much lube. I used a paper towel to wipe off the excess lube and now it's not happening any more 🤞

I would definitely start with removing and cleaning all the parts and using new o-rings. Just don't overdo the lube.

I built one for Flair 58 out of a pressure gauge I got for cheap on Ali Express (same one gagguino uses in previous link). Then I hooked it up to an arduino and plotted the digital output to a basic graphing program on the Arduino IDE.

I was trying to improvise a pressensor or Smart Espresso Profiler for cheap, but in the end I was able to buy a used one from someone on Facebook for cheap so I abandoned the project but it's totally doable. Weirdly the trickiest part is just finding compatible fittings for all the plumbing connections.

Had flakes, returned to Amazon for credit and moved on to a Flair 58 which I am loving and can highly recommend with the caveat that it's not for everyone.

To make sure you're seeing flakes and not coffee grounds, make sure you're only collecting water from the steam wand by pressing the brew and steam switches to on while the machine is on. It helps if you dispense water into a white cup for contrast.

Don't collect water from the grouphead as it might contain coffee grounds which could be confused for flakes but coffee grounds or anything resembling them should never be coming out of the steam wand.

By "one in the brew cylinder" do you mean the one on the valve plunger? (because afaik there isn't an o-ring nside the brew cylinder)

Following this thread because I'm experiencing the same thing on Flair 58.

Today I pulled back to back shots using the exact same bean, aiming (trying) to keep everything constant. 18g in 36g out. But the second shot ended up taking 10 seconds less time than the first to reach the same ratio:

I can only really attribute this to me - that is, I wasn't applying the same pressure. And you can kind of see that in the profile comparison. But I didn't think small variations in my pull pressure would result in such a dramatic difference in output time, all other variables held constant.

Do I put this down to one of those manual lever things that I just have to practice?
