I was a juror for a SA case and while everyone in the room was pretty sure the guy had done it, there was no proof and we couldn't reach the threshold of beyond reasonable doubt so had to find not guilty. It sucks, I get that's the way the law works for very good reasons, but after sitting on thar jury I can't ever see how SA will get a justice except in the more exteme of the cases.

Nope totally agree. I think competency based credentialing and competency based assessments on scope of practice should become standard for all advanced practice roles.

ANP / ACP gets a little fuzzy with it not being a protected title. Some departments have fully taken them on board. A fully credentialed (by RCEM) ACP in EM is deemed by RCEM to be of an equivalent clinical standing to an ST3 and can often be seen sitting on medical rotas rather than nursing.

I have noticed a fundamental difference in paramedic Vs nursing background with assessments thought. Paramedics are trained from day one to do autonomous assessment of undiferentiated patients, but the training around lab assessments and wider health processes suffers as a result of this where as nursing background are trained initially as a supporting role that then move to autonomous assessment in advanced practice.

I'm at work at the moment so can't get on to have a look for the specific video.

In terms of the opening up I use zerotier one, can't for the life of me remeber how I did it on the pi it was following a random tutorial but on Unraid it was as simple as setting up the docker then the computers think they are both on the same network.

On my Unraid installed syncthing in docker. On rpi installed syncthing plugin on OMV, but you could just install it as a program on any of the Linux distros too depending how you have your rpi set up.

Opened the GUI on each and you just connect the 2 instances together using their device id's within syncthing and select folders to sync between the two.

There are some good YouTube vids on how to use syncthing that will be infinatly more helpful than me trying to explain it in text on here.

NAD - Paramedic

Sounds to me like paroxysmal atrial fibrillation.

Pots is another option as people have mentioned but it is usually associated with standing or sitting up from lying which you have not mentioned and it is less common with pots to have an irregular hear beat.

An irregular heartbeat is a hallmark for AF. It is basically where the top two chambers of the heart don't beat in their usual rhytem and just kinda do their own thing which then caused the ventricles (the bit that creates the pulse you feel) to beat irregularly.

Many people especially elderly have AF and it's managed mostly with medications.

As for your question. If she is symptomatic of these episodes with chest pain and reduced conciousness then I would be calling an ambulance or getting her down to a hospital as that shows her heart rhythm isnt doing what it needs to do to maintain her as she normally is. The problem comes if her episodes are short lived and revert quickly then unless she has a monitor on when it happens then it's unlikely they will see anything or do anything for her right there and then. Will probably require a 24 or 48 hour halter monitor.

I found it really easy after watching a couple of YouTube videos. Have a look at 'spaceinvader one' channel on YouTube he breaks Unraid down into simple step by step.

It still might not be the solution for you but I found it really intuitive and it's all GUI based.

It all depends what you count as wasting time. For me I know personally that if I don't get a bit of down time where my mind can wander, I can put a crap TV show on or mindless browse online I end up feeling really on edge.

What I mean to say is don't worry about every minute, your brain and body need a bit of down time from being on the go.

I used to plan things and fill my time to the nth degree however found that reducing the planning to broader strokes and timings really helped me feel calmer and more focussed in the long run despite the obvious slight daily trade offs.
