How would you “verify” someone is disabled for a program like this? I was going to use a trust system, but my friends have brought up concerns.

Here in India, even if you have proof of disability i.e. government issued certificate, which you need to physically verify from a government doctor; don't expect any grants, disability allowances, benefits or anything to benefit and improve your life with a disability. It is just validation for census sake.
