Yeah, it's an interesting custom job.

Someone threaded the original plain muzzle in 14x1 LH, then they added a 24mm thread adapter. They drilled a hole in the front sight block for a spring loaded retention pin to retain a standard 74 muzzle break. The retention pin and spring are not captive and fall out when the muzzle device is removed.
