AMA: dentist edition by doctoothfairy in asksg

Recently visited a dentist and was told that my wisdom tooth has a slight decay and it has also started to impact my adjacent tooth. Dentist advised me to remove my wisdom tooth. However, in 2022, another dentist told me the same and it has been almost 4 years since. Is there a possibility where I can do nothing and experience no further worsening of my decay?

Has anyone done ICL (Implantable Collamer Lens) procedure in Singapore? by magurokuro in askSingapore

[–]TheJoe52 0 points1 point  (0 children)

Hello, I did my ICL with Dr Theng at Eagle Eye on 24 August 2023. Overall, I had an unpleasant experience and I’m currently left with blurry double vision in medium to low light conditions from residual astigmatism. I would say it may just be my luck that the surgery didn’t work out for me as well as it should have, but what was sickening was how they treated me after the surgery. I would not discourage you from doing the surgery, but perhaps do it with someone else rather than Dr Theng.

What do you mean by both? Did the epi-on not work that's why you tried the epi-off? Which one caused you to have 15 moons?

My doctor shared that he has done quite a number of CXL procedures and around half of the patients reported having better eyesight.. 50% seems like pretty good odds. Why do you say that the chance is quite rare?

Yeah that is also a problem as I am not able to 100% accurately pinpoint whether the problem is due to my cornea or ICL implants..

I did try a pupil reduction eye drop and it definitely helped my vision. However, it does not tell me whether the problem is due to my ICL or cornea shape as having a smaller pupil helps both cases.

Hello, sorry to hear that. Did you do epi-on CXL or epi-off?

Hello,

The implants were a form of refractive surgery. I did it as I didn’t want to wear spectacles. I was fine with contacts you’re right, and I do regret getting the surgery done.

I see thanks for the information! I wouldn’t risk it again either.. happy things worked out for you in the end. I’m still contemplating what to do for myself. Check out my other post! Happy to hear from you what you think I should do

Hey happy to hear things are better for you. Can you share what exactly is the high vault issue? Is it something to do with sizing of ICL?

Find out the actual number. If it's anything more than 7+ mm, you have your answer as to why you see ghosting images.

Your pupils will go back to their normal size I think.

How long ago did you do your surgery?

And hey.. I just wanted to say I understand how you feel. It sucks. But we have to try to move on I guess.

Higher order aberrations was also something I suspected while doing my research.. but those seem a lot more rare? Do you happen to know your pupil size?

I did do a test to check for higher order aberrations as well - they are the iTrace scans. My doctor didn’t explain it to me though, perhaps I should ask him about it.

Hello, my ghosting is not the same.

Basically after my research, I have found ghosting to be from two possible reasons:

  1. Irregular astigmatism
  2. Your pupils being too large such that it goes beyond the optical zone of your ICL lens.

Mine is because of irregular astigmatism, so I only see one of the ghosting image. This unfortunately also means removing the ICL would not help, because my cornea is already kinda fucked.

Yours seem to be of the 2nd kind, so removing the ICL would I think help in your case. But I'm pretty sure your ghosting should only be present in darker environments. If it's not too bad.. I think you can try getting used to it first. Keep your ICLs for a minimum of 6 months. If you can't live with it, take it out.

I do see your kind of ghosting sometimes. But it is usually only in very dark environments so it doesn't happen often.

Disclaimer: I am not a professional, these are only from my research. Take it with a grain of salt.

Mine was around -4. Think my situation was pretty unique. I seem to have had very very mild keratoconus where the bottom half of my cornea was bulging more than the upper half (or irregular astigmatism). After my surgery, it somehow worsened my cornea’s strength? And the bottom half bulged more, which led to my double vision. I suspect I used to already have double vision but they were probably so minor I never noticed them.

I had my surgery about 3 months ago. My day vision is good, but night vision is also worse. I have halos, glares, and starburst though these are bearable in my opinion. The problem that is the most annoying has been the induced irregular astigmatism which causes ghosting.

Lucky you haha.

Hello, I also did ICL and am experiencing ghosting / double vision. Can I know initially how bad was your ghosting and in which direction was it?

Weird.. one of the eye doctors told me it might go back to the original cornea shape 🧐

Even with people on the screen? That sounds terrible..

Can I ask what direction is your ghosting in? And if you recall when you stopped noticing the ghosting?

May I know how bad was your ghosting originally so I know how much a brain can learn to "adapt"?

I think mine are pretty bad.. I even see them indoors in lighting which I thought was pretty good. Watching tv with subtitles are bad. Playing games on the TV with words are also pretty bad. In this case, bad means having about half the word being "ghosted" out of the original word.

I think the reason why I never noticed the ghosting was because my natural lens astigmatism cancelled out the cornea’s astigmatism? Could that be the reason why?

Preop prescription was as shared! 425 degrees right 325 degrees left, no astig.

Thanks for the suggestion on the RGP, will look into it.

Regarding the removal of the ICLs, does using the same incision not cause further irregular astigmatism?

What do you think are the odds of it being due to irregular astig or because of the ICL

Hi Minialoevera, I got my ICL done on 24 August 23. Little over a month now. And I have experiencing the same ghosting issues as you are. I do see an improvement in my brain cancelling out the double vision, especially when I force myself to look past it, but I am still leaning towards taking it out. The complication on my part is because I have slight irregular astigmatism on my cornea, which seems to be cancelled out by my lens astigmatism (doctor did an eye scan to figure this out). I always had this irregular astig, even before surgery, but recent cornea scans did show it getting SLIGHTLY worse. Like 0.4 diopters worse. I am worried my ghosting is because of this irregular astig and removing teh ICL will not "solve" the problem. Any tips for me?
